Ace Medical Supplies LLC ("Ace Medical Supplies," "we," "us") is a durable medical equipment and medical supply provider located at 10002 NW 46th St, Sunrise, FL 33351. This Privacy Policy explains how we collect, use and protect information when you visit https://www.acemedicalsup.com (the "Site") or contact us through it.

When we provide medical equipment and related services to patients, health information is also protected by the Health Insurance Portability and Accountability Act (HIPAA) and described in our Notice of Privacy Practices. Information we collect

Information you give us

Referral forms: referring provider or case manager details; patient name, date of birth, contact information and delivery address; insurance or workers' compensation claim information; equipment requested; physician information; clinical details; and documents you upload, such as prescriptions, face sheets, insurance cards and medical records.
Insurance verification requests: patient name, date of birth, insurance plan and member ID, equipment needed, and your contact information.
Contact and partnership forms: your name, organization, phone number, email address and message.
Phone, fax and email: information you share when you contact us directly.

Information collected automatically

Like most websites, our hosting provider records basic technical information when you visit, such as your IP address, browser type, pages viewed and the date and time of your visit. This is used to operate, secure and improve the Site.

Health information and HIPAA

Information about a patient's health, treatment, equipment or payment for care is protected health information (PHI). We use and disclose PHI only as permitted by HIPAA and other applicable laws, as described in our Notice of Privacy Practices.

Referral and insurance forms are transmitted over encrypted connections (HTTPS).
We use only service providers that have agreed in writing to protect PHI as required by HIPAA (business associate agreements) to receive or store form submissions containing PHI.
Please do not include health information in our general contact form. Use the referral form, fax or phone instead.
If you submit a referral for someone else, you confirm that you are authorized to share their information with us for the purpose of providing medical equipment.

How we use information

To process referrals and orders, verify insurance eligibility and benefits, and obtain required documentation from physicians
To schedule delivery or shipping, set up equipment and provide training and service
To bill insurance plans, workers' compensation carriers and patients, and to collect payment
To respond to questions and communicate about orders, rentals, resupply and service

We do not sell your personal information or health information, and we do not use health information for marketing without your written authorization.

The Site uses only the cookies and similar technologies needed for it to work. We may use privacy-respecting website analytics to understand how visitors use the Site in general. We do not place advertising or third-party tracking technologies on pages where you submit health information, such as our referral and insurance verification forms.

Calls, texts and emails

If you give us your phone number or email address, we may contact you about your referral, order, delivery, insurance, billing, rentals, resupply or service. Message and data rates may apply to text messages. You can tell us at any time to stop non-essential messages; reply STOP to opt out of texts. We will never ask for your full credit card number by text message or email.

Payments

Online payments are processed by a third-party payment processor on its secure website. Your card or bank details are entered directly with that processor and are not collected or stored by our Site. The processor's own privacy policy applies to the information you give it.

Security and retention

We use administrative, technical and physical safeguards designed to protect information, including encrypted connections, access controls and staff training. No method of transmission or storage is completely secure, so we cannot guarantee absolute security. If a breach of unsecured health information occurs, we will notify affected individuals as required by law.

We keep information as long as needed for the purposes described here and as required by law, payer rules and accreditation standards, including medical record and billing retention requirements.

Your choices and rights

You may ask us to update or correct the contact information you gave us.
Patients have rights under HIPAA to access and get copies of their health information, request corrections, request restrictions, request confidential communications, receive an accounting of certain disclosures, and file a complaint. These rights are described in our Notice of Privacy Practices.
If you believe your privacy rights have been violated, you may contact us at the information below. You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.

Children

The Site is intended for adults. We do not knowingly collect personal information online from children under 13. Information about a minor patient should be submitted by a parent, guardian or the child's health care provider.
